More Than 100 NYC Adult Day-Care Providers Face Medicaid Fraud Scrutiny

NEW YORK CITY, NY –

More than 100 New York City adult day-care providers are under scrutiny in a coordinated Medicaid-fraud crackdown that is testing how aggressively the state protects money meant for vulnerable adults.

Federal, state and city investigators have spent months examining social adult day-care programs and related managed long-term care services. Officials announced this week that teams had conducted more than 80 site visits, searches or warning-letter deliveries as part of the effort.

The sweep involves the federal Centers for Medicare and Medicaid Services, the U.S. Department of Health and Human Services inspector general, the FBI, the NYPD and New York State agencies. Authorities are examining possible improper billing, kickbacks and money laundering, according to official statements and local reporting.

Those are allegations, not findings of guilt. A search, site visit or warning letter does not prove that a provider committed fraud. Some businesses identified in news reports have disputed wrongdoing, while others have not publicly responded.

The scale of the inquiry is significant. More than 400 social adult day-care programs serve roughly 45,000 Medicaid members in New York, with annual spending exceeding $500 million, according to the governor’s office. The programs are designed to provide supervision, meals, social activities and support to older adults and people with disabilities who might otherwise face isolation or institutional care.

That mission makes weak oversight especially costly. Fraudulent billing can drain public funds, but an indiscriminate crackdown can also disrupt legitimate care. Investigators and regulators must distinguish between deliberate fraud, poor recordkeeping and providers that follow the rules while serving people with complex needs.

Governor Kathy Hochul ordered the state Department of Health to recommend reforms by Dec. 31. The review could include tougher enrollment standards, stronger background checks, more frequent inspections and changes to how the program is financed or structured.

The state says the current enforcement action follows years of warnings. In 2025, officials reviewed 418 social adult day-care programs. Between 2021 and 2025, the Office of the Medicaid Inspector General investigated 387 subjects tied to the sector and referred roughly 100 matters for possible prosecution, the governor’s office said.

Those numbers raise an uncomfortable question: if investigators identified widespread concerns for years, why did the system remain vulnerable?

Medicaid fraud is often treated as a bookkeeping crime. The public cost is more direct. Every improper payment is money unavailable for home care, medical treatment, transportation or other services that help New Yorkers remain safely in their communities.

Providers also deserve a clear process. Investigators should disclose what standards were violated, which payments are disputed and whether patients will lose services. Agencies should move quickly to protect public money without leaving families to discover that a trusted program has suddenly closed.

The investigation is still active. No complete list of targeted providers or alleged losses has been released. Officials have not said how many cases may lead to civil recovery, license action or criminal charges.

The next test is accountability, not headlines. Regulators must show what failed, who benefited and how the rules will change. They must also protect the people who rely on these programs from becoming collateral damage in a system-wide cleanup.
